Beechwood Memorial values your privacy rights and takes many precautions to safeguard your health information. We want the right people to get information necessary for your care, and make your access to information as convenient as possible.

Your medical record contains documentation by all physicians and healthcare providers involved in your care and treatment while you were here at Beechwood Memorial. It contains the results of diagnostic tests and therapeutic procedures performed, as well as information about your medical history, and your condition and progress during your hospitalization.

How Does My Doctor Receive My Medical Record Information?

Our goal is to get your health information to the appropriate health care provider as quickly as possible. Your physician (or other healthcare provider) may request a copy of your medical records by faxing their request to (877) 865-9738. There is no charge to you for this service.

How Do I Receive My Medical Record Information?

Please complete the written request for records and HealthPort fee acknowledgement form: English | Spanish.

While it is not required that you use this form, it is our policy that medical record requests be submitted in writing. If you are unable to print the form from the website and would like a copy, you may pick up a paper copy in person at the Information desk in the main lobby of the hospital or at any point of registration.

Your medical record, and the information it contains, is confidential. Therefore, we must confirm your identity before releasing any records. Please be aware that only the patient or his/her personal representative can request medical records.

For your convenience there are several ways to submit your medical records request:

You may drop off the documents listed below to the Information Desk in the main lobby at Beechwood Memorial. We are available to accept medical records requests between the hours of 8:00am ” 5:00pm, Monday through Friday.

Information needed for all requests:

1) Written request for records and HealthPort fee acknowledgement form ” completed and signed: English | Spanish

2) Copy of a current government issued photo ID such as passport, driver’s license, State ID, or military ID.

You will receive your records by the method selected on your request 5-7 business days after our receipt of your request. There is charge of $0.25 per page for all requests. Once the record has been processed, you will receive an invoice for the total amount due.

If your request is for records more than a year old or for multiple or extensive stays, it may take up to 30 days to reproduce the records. Be sure to include a current phone number with area code in your written request so we may contact you if we have questions regarding your request.

How Do I Order a Birth Certificate?

Beechwood Memorial does not maintain birth certificates; you must request them from the Idaho Bureau of Vital Records and Health Statistics. Learn more about this process here.
