At EIRMC’s Transitional Care Unit (TCU), we help patients recently treated in a medical or surgical environment to become more self-sufficient before being discharged from our care. We are a 16-bed Skilled Nursing Facility (SNF), all with private beds. We offer therapy six to seven days per week, and the average length of a patient’s stay is seven to 14 days. TCU is located on EIRMC's sixth floor.

According to Medicare’s most recent ratings of skilled nursing facilities, the TCU unit received:

  • “Overall” rating of 5 out of 5 stars
  • “Nursing Home Staffing” rating of 5 out of 5 stars
  • “Quality Measures” rating of 5out of 5 stars
  • “Health Inspections” rating of 4 out of 5 stars

TCU’s accommodations are designed to promote healing in a less stressful environment and at a lower cost than rooms on other hospital floors. It's ideal for patients who no longer need acute medical care, but still need constant medical supervision. We also provide other medical services such as physical therapy or wound care.

TCU is staffed around the clock by some of the most skilled and experienced nurses we have. Our goal in the Transitional Care Unit is simple: to help you get healthy, so the move between hospital and home is as seamless as possible.
